Can a Local Longevity Clinic Compete With Fountain Life and the Funded Brands?

What the National Players Actually Do, Where They Don't Reach, and How a Local Clinic Can Win

The Objection Many Prospective Owners Raise

Somewhere in the evaluation process, many entrepreneurs looking at the longevity category ask the same question: how do I compete with the funded national brands?

It is a fair question, and the answer is more favorable than it looks, but only if you understand what those brands actually are and where they actually operate.

If you are weighing a franchise instead, see our guide to longevity clinic franchise options in 2027.

This post is educational and does not constitute legal or financial advice. Apart from ACG's own planning estimate, figures cited come from public reporting and federal data and are dated where they appear. If you are weighing a franchise, request the current Franchise Disclosure Document directly from the franchisor and have it reviewed by a franchise attorney and an accountant before signing anything.

What the National Brands Actually Are

Fountain Life, co-founded by Peter Diamandis, Tony Robbins, Robert Hariri and William Kapp, raised an $18 million round reported in 2025, bringing total funding to roughly $108 million. Reporting at the time described existing locations in Naples, Orlando, Dallas and Westchester, and expansion into Houston, Los Angeles and Miami.

Its model combines advanced diagnostics with AI, integrating genomics, imaging and metabolomics into a subscription-based membership program. Its full subscription was reported at $21,500 a year in August 2025.

Trade coverage describes a broader field, among them Biograph and the franchises Next Health, Humanaut and Upgrade Labs, in what one publication called a clinic arms race.

Read that carefully and the competitive picture becomes clearer. The premium brands, Fountain Life among them, are diagnostics-led operations concentrated in a small number of major metros, built around high-ticket programs for a high-net-worth membership.

Where They Do Not Reach

The United States has 387 metropolitan statistical areas in OMB's current delineation (July 2023), so a handful of locations leaves many of them without a national longevity brand presence.

More importantly, the reported $21,500-per-year membership addresses a narrow slice of the market. The adult in a mid-size city with above-median household income who wants hormone optimization, metabolic support, and regular biomarker tracking is not the customer for that offer. There are vastly more of those patients than there are candidates for a premium concierge membership, and in many markets nobody is serving them well.

That patient is described in detail in our post on longevity clinic patient demographics.

The Four Advantages a Local Independent Clinic Holds

Price positioning

An independent clinic can build a membership at a fraction of a premium national offer, because its cost structure is lower. A clinic with no royalty and no brand fund can price for its market rather than for a national brand's cost structure. ACG's planning estimate for the startup investment in any of the 13 clinic types we support is about $107,000 to $130,000, our consulting fee included; working capital sits above it. Pricing for our engagement itself is discussed once the scope is clear.

The relationship

National diagnostics-led models are built around periodic assessments. A local clinic can offer an ongoing clinical relationship with the same provider, which can drive retention in this category. Many patients stay for the relationship and the monitoring, not for the scan.

Local search

Someone searching for a longevity clinic in a specific city is looking for a local provider. A well-executed local search presence can beat national brand awareness for that query, and national brands do not compete for it in markets where they have no location. See how ACG audits and plans local SEO for a longevity clinic. Marketing execution is a separate engagement.

Service flexibility

An independent owner can adjust the service menu to what the market actually wants, with the medical director signing off on each change. A franchised or corporate location generally cannot.

What the National Brands Are Actually Doing for You

There is an underappreciated upside to all of this activity. Funded national brands are spending heavily to educate the market about what a longevity clinic is and why someone would join one.

Every podcast appearance, every press cycle, every ad campaign expands the number of people who understand the category and are looking for it. Many of those people do not live near a national location and could not afford one if they did. They search locally instead.

A category with well-funded brands building consumer awareness is a better market to enter than one nobody has heard of. The awareness is being paid for by someone else.

What to Do If One Opens Near You

If a funded brand or a franchised location does announce in your market, the response that works is not to match them.

If their price point sits above yours, as a premium brand's does, the segment they take is the one you were least likely to serve. The patients who remain are choosing on access, relationship, and value rather than on the most extensive diagnostic panel available.

Three practical moves. Sharpen the price positioning explicitly rather than leaving it implied, so a prospective patient comparing you sees the distinction immediately. Deepen the clinical relationship, which is the thing a periodic-assessment model structurally cannot match. And invest in the local search and referral presence that a new entrant has to build from scratch while you already have it.

A new competitor also validates the category locally. Patients who learn what a longevity clinic is from their marketing will search, and that search is often local and generic rather than branded.

Where You Should Not Compete

Attempting to match a national brand on diagnostic breadth is the wrong strategy. Whole-body imaging suites and genomics platforms require capital an independent clinic should not spend chasing a competitor who is not in its market.

The right position is a comprehensive, relationship-driven clinical program priced for the local market, with biomarker monitoring that is genuinely useful rather than maximally extensive. If that is the clinic you want to build, our team can help you launch an independent clinic in your own market; our longevity clinic page covers what it takes to open a longevity clinic.

Frequently Asked Questions

What does Fountain Life charge?

Fountain Life's membership page lists its tiers without prices. In July 2026, Longevity.Technology reported a $595-a-year BASE membership from Fountain Life, covering blood testing, a DEXA scan at one of its centers, a clinician's review of results and a year of digital health tracking. The company's comprehensive annual subscription was reported at $21,500 in August 2025, and a testing-only package at $10,500. The full programs define a narrow, premium segment, and BASE is a testing and tracking membership, which leaves the ongoing local clinical relationship open.

Should I worry about a national brand opening in my market?

It is worth monitoring. The premium brands' announced expansion is concentrated in large metros, so in a mid-size market the practical risk from them is lower, but franchised brands also open in smaller markets, as Next Health did in Boulder, Colorado, in March 2026. If you are in a target metro, differentiate on price positioning and clinical relationship rather than attempting to match diagnostic breadth.

Do I need genomics and advanced imaging to be credible?

No. Credibility in this category comes from a thorough clinical process, meaningful biomarker monitoring, and a provider relationship patients trust. Those are achievable without an imaging suite or a genomics platform, and they can drive retention more reliably than the scan does.

Does a national brand entering my market hurt my pricing?

Usually the opposite for a premium brand. Its full programs generally price well above an independent clinic's, which makes your value positioning clearer rather than weaker, and the segment it captures is the one least likely to have chosen you regardless. A franchised location may price closer to yours, so compete there on the clinical relationship and your local presence.

Is the longevity market getting saturated?

Not at the local level in much of the country. The premium brands, Fountain Life among them, sell their full programs at premium prices in a small number of major metros, and the franchises are still adding locations. The mid-market, mid-size-city opportunity remains broadly unserved, which is where many independent clinics operate.
