Med Spa Consulting, From Ownership
Rules to the Service Menu.

A med spa is a medical practice that looks like a spa, and it is regulated as a medical practice. Our med spa consultants help you get the ownership, supervision and service menu right before you sign a lease, or fix what holds a running med spa back.

What a Med Spa Is

What a Med Spa Is,
and Why It Is Harder to Run Than It Looks

The American Med Spa Association describes a medical spa as a blend of an aesthetic medical center and a day spa: non-invasive, non-surgical aesthetic medical treatments under the oversight of a licensed physician or independent practitioner, delivered by skilled health care professionals in a spa-like setting. U.S. searches for “med spa near me” run 100,000 to 1 million a month (Google Keyword Planner).

What Patients See
  • Neurotoxins and dermal fillers
  • Laser and energy-based skin treatments
  • Chemical peels and microneedling
  • Body contouring and medical-grade skincare
  • Hydration IVs, hormone and weight loss programs as the practice matures
What Runs Behind It
  • Injectables are prescription products
  • Lasers carry state supervision rules
  • Every treatment needs a provider your state allows to perform it
  • A Medical Director who directs all clinical care
  • Ownership, sourcing and advertising rules that differ by state

Every client works with a team of our clinic consultants, not a single consultant, from your first strategy call through launch or your growth plan.

The team sets your brand positioning, core offer and go-to-market plan and covers marketing strategy, operations and launch. Meet the clinic consultants on our team.

Where Med Spas Run Into Trouble

The Med Spa Rules
Our Consultants Plan Around

Ownership, supervision, sourcing and advertising rules differ by state. Here is where they stand in October 2026.

Who May Own It

Several states require a physician to own the practice or a management-company structure. We structure for yours, with your healthcare counsel.

Who May Treat, and Under Whose Supervision

Injector and aesthetician scope of practice follows state rules, and so does who must examine the patient and supervise injectables and laser treatments.

Where Your Products Come From

Federal law requires providers who dispense or administer prescription drugs to buy them only from authorized sources. In 2024 the FDA warned that counterfeit Botox had been found in multiple states, apparently bought from unlicensed sources. Supplier introductions are made at no additional charge through our clinic supplier network.

What You May Sell and Advertise

Not every product or use on a med spa menu is FDA-approved. A compliant GLP-1 program runs on FDA-approved products; compounding belongs only inside an FDA-regulated study (an IRB-approved protocol and an IND), and a study drug is never promoted or sold commercially. Exosomes for therapeutic use are not FDA-approved at all, and we steer you away from them. LegitScript certification affects where you can advertise.

Current as of October 2026. Confirm with your Medical Director and qualified counsel; this is not legal or medical advice.

Who This Model Is Built For

Is a Med Spa Right for You?

A Med Spa Fits You If
  • You want to own the business and build a brand patients choose by name
  • You would rather keep your brand and pay no royalties than buy a franchise, which typically charges royalties of 5 to 8% of revenue for the life of the agreement (compare independent clinic vs. franchise)
  • You want the compliance right before opening day, not after the first letter from a state board
What It Takes
  • A structure your state allows: some states require physician ownership or a management company
  • Startup capital: ACG's planning estimate for the startup investment in any of the 13 clinic types we support is about $107,000 to $130,000, our consulting fee included; working capital sits above it.
  • Pricing for our engagement itself is discussed once the scope is clear.

View All FAQs
What does a med spa consultant do?+
A med spa consultant helps you plan the parts of a med spa that medical rules govern: the ownership structure your state requires, the Medical Director and supervision arrangement, who may perform each treatment, where prescription products come from, and a service menu that fits your market. At ACG that is either the five-phase launch of a new med spa or, for a med spa already open, an assessment followed by prioritized findings on training, suppliers, operations and marketing. Marketing execution is a separate engagement.
Do I need a med spa consultant?+
No. Many med spas open without one. A consultant helps most where med spas run into trouble: ownership rules, supervision of injectables and lasers, product sourcing, and an equipment budget that fits your service mix. Ask any firm whether its team has opened med spas, who leads the engagement, what the deliverables are and how the fee works, in writing. Our med spa consulting covers each of these; every engagement is run by a team, not a single consultant, and you can meet the clinic consultants who work on it.
Who can own a med spa?+
It depends on the state. In most states, no medical background is required to own the business; an independent, state-licensed Medical Director directs all clinical care. Some states require physician ownership or a management-company structure, and ACG structures for yours with your healthcare counsel.
Who can perform Botox and fillers at a med spa?+
Only providers your state allows, under the supervision your state requires; the rules for physicians, nurse practitioners, physician assistants, registered nurses and aestheticians differ by state. Your Medical Director sets the protocols, and we map the rules for your state before you hire.
What startup investment should I plan for a med spa?+
ACG's planning estimate for the startup investment in any of the 13 clinic types we support is about $107,000 to $130,000, our consulting fee included; working capital sits above it. Pricing for our engagement itself is discussed once the scope is clear.
How long does a med spa engagement take?+
For a new med spa, ACG targets 60 days from receiving your clinic keys to your first patient, market and regulatory conditions permitting; your state's ownership rules, licensing and equipment lead times set the real pace. For a med spa already open, the assessment comes first, and its findings set the plan.
Is ACG a med spa franchise?+
No. ACG is a consulting and advisory firm. Your med spa carries your own brand, and you pay no royalties or franchise fees.
Does ACG provide medical services?+
No. ACG is a consulting and advisory firm, not a medical practice, and it does not provide care, prescribe or dispense. An independent, state-licensed Medical Director directs all clinical care, and ACG introduces you to vetted Medical Directors through partner networks that cover all 50 states.

ACG is a consulting and advisory firm, not a medical practice; an independent, state-licensed Medical Director directs all clinical care. Results are not guarantees of income. Individual outcomes vary. The 60-day target begins from clinic keys received, market and regulatory conditions permitting.

Built for Compliance

Structured to keep you on the right side of the line.

Knowing the rules is the foundation of every clinic we help launch.

Your Medical Director Directs Care

You own and run the business. An independent, state-licensed Medical Director makes every clinical, prescribing and treatment decision, structured for your state's Corporate Practice of Medicine laws. We connect you to a vetted network; you retain your own provider.

We Know the Regulatory Lines

PRP, peptides, GLP-1 medications and cellular products each carry federal and state rules, and not every product or use is FDA-approved. Exosomes for therapeutic use are not FDA-approved at all, and we steer you away from them. Your menu and marketing are built to hold up to FDA, FTC and state medical board scrutiny.

No Hype. No Guarantees.

We show real ranges and never promise an income or a clinical outcome. What you earn depends on your market, your capital and your execution.

Altos Consulting Group is a business consulting and advisory firm, not a medical practice, and does not provide, prescribe or sell any drug, biologic, device or therapy. The FDA has stated there are currently no FDA-approved exosome products for the treatment of any disease; clinic owners are responsible for the compliance of every product, therapy and claim.
